Teacher Collection Request FormThe Youth Services and Branch staff will be happy to put together a collection of materials on a specific topic for you. We ask that you allow a minimum of three-five (3-5) days for us to gather the materials. You may mail in a teacher collection form, call in your request to the library location where you wish to pick up your materials, or complete the form below and click on submit. Specific titles may be requested, but will be subject to availability at the time you make your request. We can put specific titles on hold, however, we cannot guarantee that they will be available within the three days that we require to pull a teacher collection. We ask that you respect the staff time it takes to pull materials to fill your request. Teacher collection privileges will be suspended for the remainder of the school year after the third time you fail to pick up a collection of materials. All materials borrowed circulate for 21 days and may be renewed up to three times provided there are no holds on the materials. For your convenience, materials may be returned to any library location plus you may renew your collection by phone. If you are returning a collection of overdue materials at the same time you are picking up new materials, please stop at the circulation desk with the overdue items and ask to have them checked in immediately..,5 overdue items will block your card. This procedure will save you time when you check out your new teacher collection. You must bring your library card to check out items, pay library fines, place items on hold or access your library account. If your library card is lost/stolen, report it to the library immediately. You are financially responsible for any materials, fines or fees attached to the account prior to the card being reported lost or stolen.
